AFFIRMOLOGY
Investment Election
Prepared for Norman Adams · Affirmology LLC · Confidential · v3, July 2026
One page, one decision. Check the amount and the structure you prefer, sign, and the full subscription packet (investor questionnaire + subscription agreement) is completed to match. Ownership below reflects founding-investor pricing: bonus units for the first check into Round 1, valid through July 31, 2026. Standard terms are 1% per $15,000 at a $1,500,000 post-money valuation (see the term sheet).
Step 1. The amount
| Choose | Investment | Your ownership (founding investor) | What it funds |
| ☐ | $30,000 | 2.5% | The fallback: fully to launch and first revenue, running lean. |
| ☐ | $40,000 | 3.5% | Launch plus a partial scaling budget. |
| ☐ | $50,000 | 5.0% | The primary ask: launch, then immediately press on what works. |
| ☐ | $75,000 | 7.5% | Launch plus real ad scaling, event staffing, and an assistant. |
| ☐ | $100,000 | 10.0% | The full plan: events fully funded, app-ops retainer, deeper ad and creator spend. |
| ☐ | Other $__________ | per the term sheet | Any amount from the $5,000 minimum. |
Every level also includes the Coherence strategic partnership package: Coherence as a sponsor at Affirmology events, mini-retreats and facilitator gatherings hosted at your center starting this fall, and cross-promotion to our audience, with specifics shaped together.
Step 2. The structure
| Choose | Structure | What it means |
| ☐ | Membership units | Real ownership in AFFIRMOLOGY LLC from signing: distribution rights, pro-rata and tag-along protections, a Schedule K-1 each year. |
| ☐ | Promissory note | Pure debt instead of equity: a fixed return of roughly 4 to 6%, repaid over 18 to 24 months, a Form 1099-INT, no K-1. |
